Changing Futures
The Changing Futures project within TWC was launched in March 2022. It’s vision is to support people with multiple complex disadvantages, including substance misuse, mental health, domestic violence, homelessness and criminal justice. In Changing Futures we take a multi-agency approach. This means we work closely with external and partner organisations to ensure that each beneficiary of the project is accessing all support available for their needs.
Changing Futures Navigators lead with lived experience. Focused on building relationships and gaining the trust of a beneficiary before any extensive work takes place, there is no time limit to how long we must work with each person. The job is to navigate the person we support through the services they require. We identify the needs of the person, whether that be support from drug and alcohol services, probation etc, but also the hobbies and interests that have been lost through their suffering of multiple disadvantages. So, a day could be to support someone to a Mental Health appointment but then take them for some food or a game of pool afterwards. Changing Futures thrives on making someone’s day better, whatever that may look like for them.
We have had many success stories over the course of the project, leading to that beneficiary being removed from our caseload due to their needs being met. During this time, we try our best to signpost to other projects within The Well Communities if the beneficiary has fewer complex needs. This is to continue their journey with the wider organisation and allow them access to further support.
Some successes include:
Supporting individuals into a detox rehab service
Supporting vulnerable women into secure women’s units
Supporting individuals to consistently make appointments
Supporting individuals to enter into a safe house
Anything that makes a beneficiary’s day better is a success.
Changing Futures 2025 is funded by MHCLG - Ministry of Housing Communities and Government, allowing us to continue supporting those with multiple disadvantages, and lead with the true, caring nature that The Well Community staff provide.
